Checklist for Working with Nonhuman Primates

The following requirements should be completed in the order listed:

□          Print off and complete Risk Assessment Questionnaire.  After completion, send through campus mail to the Occupational Health Clinic, PO Box 21-0095.

□          Make an appointment at the Occupational Health Clinic for a TB test (626-6363).

□          The following web based courses must be completed:

                        □          Laws & Regulations

                        □          Introduction to the Animal Hazards Program

    □          Nonhuman Primates Module

                        □          Surgery and Anesthesia

                        □          Zoonotic Diseases

□          After the above have been completed, a Protocol or Protocol Addendum Form must be submitted and approved by the Institutional Animal Care and Use Committee to add your name.  For a copy of the form:  http://www.uac.arizona.edu/forms/uacformspage.shtml

□          After all of the above have been completed, contact Dr. Rand at mrand@email.arizona.edu to be scheduled for the Herpes B Virus Prevention Training Course.

□          After all of the above have been completed, contact Cheryl Johnson at caj@email.arizona.edu to be scheduled for the Primate Hands-On Training Course.
