Important Notice: Changes to Requirement in documentation section on IACUC Animal Protocol Review Form.  Click here to read memo.

In order to obtain security access to the facility you must have IACUC certification, complete and submit a Risk Assessment Questionnaire to Occupational Health, and be on an approved protocol.  Visit Occupational Health website to complete form online.

As of Prior to initiation of any project, class, testing procedure, or any other use of animals, the IACUC must review and approve the use of the animals. The first step in this process is to obtain an ANIMAL PROTOCOL REVIEW FORM (APRF).  There are two options for obtaining and filling out forms: 1. Pick up a form from the Protocol Office (CAF, rm. 112, 621-9305 or 621-1330, or AHSC office, Rm. 1126), 2. You can also download the electronic version (see below). The APRF, along with any necessary addenda or documents - if applicable, are to be completed by the investigator and submitted to the Protocol Office. All forms must be typed and signed.  When a sponsored research project is involved, the investigator brings the grant proposal, routing sheet and the completed APRF to the Protocol Office.  The office, upon request, will then give the investigator a Verification of IACUC Review to submit to the granting agency. Protocol office is located in the Central Animal Facility, P.O. Box  210101,Tucson, AZ  85721, phone: 520-621-9305. Questions regarding IACUC protocols and reviews should be directed to IACUC Program Coordinator at 520-621-9305. Approval by the IACUC does not, in and of itself, constitute approval for implementation of this project. Other levels of review and approval may be required (e.g. Risk Management and Radiation Safety), and the project should not begin until all required approvals have been obtained.

The NIH, PHS, American Heart Association and its affiliates, the Flynn Foundation, ADCRC, and many other granting agencies will not accept proposals without verification of IACUC review.  In most cases, the actual IACUC review occurs after the proposal is submitted to the granting agency. The initial Verification Form states that the grant has been received for IACUC review, and that results will be submitted within 60 days. Once review is complete, the IACUC sends the Final Verification of IACUC Review to the investigator, who then submits the Verification to the granting agency.  Call the Protocol Office 621-9305 for more information on verification forms.

The Protocol Review Flow Chart provides a description of the Review Process. Following the Flow Chart, a description of terms and basic instructions on how to fill out the protocol is included.

Who Can I Contact for Help?
Contact the IACUC Office at 621-9305 for assistance with any questions concerning completion of the forms, what forms are needed and when, and deadline information. A Veterinarian from University Animal Care (UAC) should be consulted in the planning of procedures involving more than minimal pain and/or distress (e.g. surgery, death as an endpoint, restraint, etc.). UAC can be reached at 626-6702.

Additional information can be obtained by calling the Protocol Office (621-9305).  Click here for a link to frequently asked questions regarding protocol review.

It is highly recommended that when accessing non-html file links, such as .doc, wpd, .PDF formats, etc. that you 'right click' on the appropriate hyperlinks and select 'save target as..' to save these files directly to your hard drive.  Do not download forms directly from Netscape (or any other browser) because the format of the application will be changed. Please note: Depending on the software version you will be using, there may be formatting changes after saving and opening the file.
If you need help downloading the forms call Karelia Coronado at 626-1028.
