Animal Protocol Review forms and documents

ANIMAL PROTOCOL FORMS AND DOCUMENTS

Prior to initiation of any project, class, testing procedure, or any other use of animals, the IACUC must review and approve the use of the animals. The first step in this process is to submit an Animal Protocol Review Form (Protocol) along with any necessary addenda to the IACUC Office. When the Protocol is approved the IACUC Office sends the Final Verification of IACUC Review to the investigator, who then submits the Verification memo to the granting agency (if applicable).

Who Can I Contact for Help? Contact the IACUC Office at 621-9305 or 626-1247 for assistance with completion of the forms, what forms are needed and when, and deadline information. A Veterinarian from University Animal Care (UAC) should be consulted in the planning of procedures involving more than minimal pain and/or distress (e.g. surgery, death as an endpoint, restraint, etc.). Please note: The .PDF format is the preferred version as it is more concise when printed. Please submit completed forms by email attachment to: iacuc@email.arizona.edu. You must have the latest version of the Acrobat Reader. Click on the adobe icon to download. The acrobat reader is free.

It is highly recommended that when accessing the Word or PDF forms that you 'right click' on the appropriate hyperlink and select 'save target as...' to save these files directly to your hard drive. Do not download forms directly from Netscape (or any other browser) because the format of the application will be changed. Please call 626-1247 if you need help downloading the forms.
