As of January 1, 2006, in order to obtain security access to the facility you must have IACUC certification, complete and submit a Risk Assessment Questionnaire to Occupational Health, and be on an approved protocol.
As of Prior to initiation of any project, class, testing procedure, or any other use of animals, the IACUC must review and approve the use of the animals. The first step in this process is to obtain an ANIMAL PROTOCOL REVIEW FORM (APRF). There are two options for obtaining and filling out forms: 1. Pick up a form from the Protocol Office (CAF, rm. 112, 621-9305 or 621-1330, or AHSC office, rm. 1126), 2. You can also download the electronic version (see below). The APRF, along with any necessary addenda or documents - if applicable, are to be completed by the investigator and submitted to the Protocol Office. All forms must be typed and signed. When a sponsored research project is involved, the investigator brings the grant proposal, routing sheet and the completed APRF to the Protocol Office. The office, upon request, will then give the investigator a Verification of IACUC Review to submit to the granting agency. Protocol office is located in the Central Animal Facility, P.O. The NIH, PHS, American Heart Association and its affiliates, The Flynn Foundation, ADCRC, and many other granting agencies will not accept proposals without verification of IACUC review. In most cases, the actual IACUC review occurs after the proposal is submitted to the granting agency. The initial Verification Form states that the grant has been received for IACUC review, and that results will be submitted within 60 days. Once review is complete, the IACUC sends the Final Verification of IACUC Review to the investigator, who then submits the Verification to the granting agency.
